Q.1     What is a bursa?

A.1     Bursae are fluid-filled sacs (or sac-like cavities) that protects bones, tendons, and ligaments. They are lined with a connective tissue membrane called the synovial membrane and they contain a lubricating substance called synovial fluid, which provides cushioning. A.2     Bursitis is inflammation of the bursa leading to a painful swelling and restricted movement of the involved joint. Most common sites for bursitis is Elbow (Olecranon bursitis), Hip (Trochanteric bursitis), Shoulder (Sub-acromial bursitis), Heel (Retrocalcaneal bursitis and Knee (Prepatellar bursitis, infrapatellar bursitis, pes – anserinus bursitis).

A.3     The most common causes of bursitis are injury or overuse, but it can also be caused by infection, hyperuricemia or inflammatory disorders.

 

Q.4     What are the common types of knee bursitis?

A.4     Anserine bursitis - It affects the pes anserinus bursa which is located between the hamstring tendons and medial collateral ligament on the lower inner side of the knee. It is the largest knee bursa.

          Infrapatellar bursitis - It affects the bursa located just under the kneecap. It is also known as Clergyman’s knee. It can be associated with patellar tendonitis.

          Prepatellar bursitis – It affects the bursa located between the skin and patella (kneecap) bone. It is also known as Housemaid’s knee.

 

Q.5     What is the most common form of bursitis of the knee?

A.5     Prepatellar bursitis is the most common form of bursitis of the knee.

 

Q.6     What is the first line of treatment of bursitis?

A.6     For non infective bursitis, first line of treatment consists of rest to the part, treatment of underlying problem and a course of anti inflammatory medications. For Infective bursitis, treatment consists of a course of antibiotics.

 

Q.7     What is the line of treatment of refractory cases?

A.7     Refractory cases are generally treatment with local infiltration of cortisone and local anaesthetic. In a few cases, they may require surgical treatment for cure of symptoms.
